(1) Yr wyf drwy hyn yn gwneud cais i ddod yn aelod o'r Elusen
I hereby apply to become a member of the Charity
(2) Yr wyf yn cadarnhau fy mod ar Restr Etholiadol tref Cricieth ac yr wyf drwy hyn yn awdurdodi'r Elusen i gynnal unrhyw wiriadau maent yn eu hystyried yn briodol i gadarnhau hyn.
I confirm that I am on the Electoral Roll for the town of Criccieth and hereby authorise the Charity to carry out such checks as it considers appropriate to verify this.
(3) Yr wyf yn cadarnhau bod gennyf ddiddordeb mewn hyrwyddo dibenion yr Elusen sef darparu a chynnal neuadd bentref i'w defnyddio gan drigolion tref Cricieth heb wahaniaethu rhwng barn wleidyddol, grefyddol neu farn arall gan gynnwys defnyddio’r neuadd ar gyfer cyfarfodydd, darlithoedd a dosbarthiadau, ac ar gyfer mathau eraill o adloniant a gweithgareddau amser hamdden, gyda'r nod o wella ansawdd bywyd y cyfryw drigolion a'i bod yn ddyletswydd arnaf i ddefnyddio fy hawliau fel aelod o'r Elusen yn y modd a benderfynaf yn ddidwyll a diffuant a fyddai fwyaf tebygol o hyrwyddo dibenion yr Elusen.
I confirm that I am interested in furthering the purposes of the Charity namely the provision and maintenance of a village hall for the use of the inhabitants of the Town of Criccieth without distinction of political, religious or other opinions including use for meetings, lectures and classes, and for other forms of recreation and leisure time occupation, with the object of improving the conditions of life for the said inhabitants and that it is my duty to exercise my powers as a member of the Charity in the way I decide in good faith would be most likely to further the purposes of the Charity.
